Accommodation - As a DD CSM we would aid in any and all accommodations necessary in the individuals life if it is contained wtihin their IPOS - (Individual Plan of Service)
Transition - As a DD CSM transition includes moving from one environment to another - graduating from school to the next step of their dreams - closing their case through CMH and linking and coordinating services within the community
Independent Living Skills - As a DD CSM this would mean having Home Help through DHS and CLS through CMH
Job Shadow - Having a job coach on the work site observing
Supported Employment - As a DD CSM - supported employment is something that we would authorize within the IPOS - Individual Plan of Service
Supported Employment Services - As a DD CSM - Supported Employment Services would mean facilities such as ROOC and or MRS
Competitive Employment - As a DD CSM Competitive Employment is when the individual is seeking and or has established community employment and is working at their job independently without the assistance of a job coach.
Developmental Disability - Severe and or chronic conditions that result from mental and or physical impairment (M.R. - epilepsy, autism, C.P., etc) which occur before the age of 22, and are likely to continue indefinitely. The developmental disability has to result with severe functional limitations within a minimum of 3 of the following areas: Self-Care, Receptive and Expressive Language, Learning, Mobility, Self-Direction, Capacity for Independent Living and Economic Self-Sufficiency.
Person Center Planning - As a DD CSM we no longer use Person Center Planning - it was changed to IPOS - Individual Plan of Service.
Individual Plan of Service (IPOS) - Same meaning as when PCP was used - Essentially a contract that lists out all the individuals dreams and goals they'd like to have worked on - and how as well as who will be involved to achieve it. The plan is done on a yearly basis - a bi-annual is conducted at the 6 month period to review the progress and make changes if necessary. All dreams and goals listed in the IPOS are to be geared towards their mental health in order for them to have the scope and objective incorporated. An IPOS can be done any time the individual and or guardian requests.
Pre-Plan - The Pre-Plan is done prior to the individuals IPOS - this is where the individual talks about what they want to have done at their IPOS meeting - they can invite whom ever they so choose - and it also is a time when the individual can go over things that they do and do not want to have talked about.
CSM / SC - CSM is a Case Support Manager - SC is a Supports Coordinator - easiest definition - linking and coordinating services - conducting face to face contacts - advocating for individuals rights/dreams and goals and ensuring that those dreams and goals are being addressed / monitored and if necessary altered with an Addendum.
